Welcome to Spine & Sport Health!
To help us better understand your health needs, we’ll ask you to complete a patient questionnaire and medical history before your first visit. Save time by filling out the paperwork in advance. Simply download and print the forms using the link below and bring them with you to your appointment for a faster check-in.

Planning For Your First Visit
1. Health History
New patients should allow 45–60 minutes for the first visit, which includes a detailed health history to identify the root cause—not just the symptoms.
2. Examination
The doctor will examine your spine, posture, and alignment, using orthopedic and neurological tests to assess affected tissues. Additional areas, such as shoulders, knees, or feet, may be checked based on your concerns.
3. X-rays
Your specific condition may require us to take x-rays to either rule out more serious conditions or help in developing the most effective treatment plan for you.
4. Treatment
After your exam and any necessary studies, you may receive your first treatment, which could include spinal adjustments or therapy for comfort. The doctor will decide if treatment is appropriate during your initial visit.
5. Self-Treatment Plan
Before you leave, you’ll receive self-care tips such as using ice or heat, avoiding certain activities, and doing recommended stretches or exercises to ease pain and promote healing.
6. Follow-Up
You’ll receive a follow-up appointment, usually within 1–2 days, for a full report of findings. The doctor will explain what’s out of alignment, recommended treatments, and options for recovery. Significant others are welcome to attend.
Payment Options

Insurance
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and Health Partners, Cigna, and Blue Cross Blue Shield. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge.
No Insurance
For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged!
No insurance? No chiropractic benefits? You can still get the care you need. Many patients choose to pay directly because chiropractic care is both affordable and cost-effective. To learn more about our flexible payment options, give us a call anytime.
